| Liability, Waiver and Release Information Registration indicates agreement by the Exhibiting Vendor to assume responsibility and to hereby release ARK Enterprises, LLC, Hilton Hotels, Exhibits, Inc., and VAE (Visual Aids Electronics), (the “Released Parties”) from all liability as a result of its actions or inactions during the event. The Exhibiting Vendor shall indemnify, save and hold harmless the Released Parties from and against, any and all claims, losses and damages, injuries, awards, fines, government charges and liability - together with all costs, expenses, and reasonable attorneys fees in connection with its display, participation or presence at the Conference, Conference activities and Exposition. Certificate of Insurance Request Exhibitors shall provide a Certificate of Insurance for the Utility Payment Conference with a 30-day notice of cancellation provision effective for October 26 - 29, 2009. The Certificate shall be issued for at least $1,000,000 per occurrence, $1,000,000 aggregate and shall cover worker's compensation, commercial general liability, independent contractors, personal injury and blanket contractual liability insurance. Additional Insured should be listed as: ARK Enterprises, LLC d/b/a Utility Payment Conference, Hilton Hotels, Exhibits, Inc., and VAE (Visual Aids Electronics). The Certificate of Insurance must be received by the Utility Payment Conference no later than September 1, 2009. Standish Ave., Anaheim, CA 92806-4319 Labor The Exhibitor agrees to comply with all union regulations and to abide by all agreements between the Utility Payment Conference and its contractors, including the Hotel, the Trade Show Service Provider or any of its agents. Cancellation and Change Policy In addition, our/my submitted registration form indicates we/I understand the Utility Payment Conference Cancellation and Change Policy and accept its’ terms as follows: 1. 100% refund until August 1 2. 50% refund until August 15 3. No refund after September 1 4. Changes and cancellations must be made via email to info@paymentconference.com 5. A $50 processing fee will be applied to all cancellations 6. An initial change fee will be waived 7. Each additional change will incur an administration fee of $50 |
